Direct Primary Care Disclosure

Magnus Direct Primary Care, LLC is a direct primary care/direct health care membership practice.
Under the direct primary care model, patients pay a recurring membership fee for access to certain primary care services described in the membership agreement.
Important Insurance Disclosure
Membership with Magnus Direct Primary Care is not health insurance.
Magnus DPC does not bill health insurance for services covered by the direct primary care membership agreement.
The membership agreement does not pay for or reimburse services provided by third parties.
Membership does not replace health insurance, Medicare, Medicaid, workers’ compensation insurance, hospital coverage, emergency coverage, specialist care, catastrophic coverage, or other insurance coverage.
Patients remain responsible for medical services and expenses not covered by the membership agreement.
Services Not Covered by Membership
Unless specifically stated in the membership agreement, patients are responsible for outside services and costs, including but not limited to:
Emergency room care.
Hospital care.
Specialist care.
Outside laboratory charges.
Imaging studies.
Prescription medications.
Medical equipment.
Outside procedures.
Pathology fees.
Vaccines or medications not stocked by the practice.
Partner services.
Aesthetic services.
Wellness services not included in the membership.
Hormone therapy, peptide therapy, injections, weight management medication, supplements, and other separately priced services.
Florida Direct Health Care Agreement Disclosure
The direct primary care agreement itself contains the legally required Florida disclosure language.
For clarity, patients should understand:
The agreement is not health insurance.
Magnus DPC will not file claims against a patient’s health insurance policy or plan for reimbursement of health care services covered by the agreement.
The agreement does not qualify as minimum essential coverage.
The agreement is not workers’ compensation insurance and does not replace an employer’s workers’ compensation obligations.
